Those clients who are leaning towards FUE hair transplant there are advantages and disadvantages that needs to be discussed before the procedure is recommended. The advantage for this method over strip or linear extraction is there is less visible scarring in the donor area after the surgery. So if a patient keeps their hair very short this may be preferred method of harvesting the grafts. However, since this is blind extraction there are many disadvantages. One of the disadvantage to a blind site incision in the donor area is collateral damage to the existing donor hair. This could cause damage to the donor site and also the hair follicles being extracted, thereby resulting in poor growth and limiting the donor hair available for future use. If a larger number of grafts need to be excited from the donor area, there is a possibility of donor hair depletion in the back of the head which leaves the hair in that area very thin and uneven. In addition, since hair loss is progressive, if a client is young and continues to lose hair, then the donor hair will be limited for any secondary procedure in the future. Also clients with curly hair, a patient with more advanced hair loss or a patient with limited donor hair availability, this is not recommended. Other disadvantages to FUE hair transplant is longer time for donor harvesting, higher expense, donor area shaving and multiple white spots formation in the donor area.

If the patient is a good candidate for FUT or the strip method, there are new techniques developed that could minimize the scarring and almost invisible to find after healing. In recent years a more advanced procedure for closure of the wound was developed by hair transplant surgeons for harvesting the grafts. This method is called the Trichophytic closure, a superior and exclusive method in camouflaging scaring or removing the previous scars that were there from other hair restoration procedures. This methods is a very meticulous donor removing and closure technique. The lower edge of the incision is removed in such a manner that the hair grows through the incision making it very hard to see after it heals, even with a short hair cut. As a result, in most cases the trichophytic method allows hair transplant patients to wear short hairstyles without the fear of exposing a surgical scar. This FUT hair transplant harvesting techniques allows for removal of more grafts for mega sessions, and no collateral damage to the existing donor hair. It addition if a client needs additional procedures later on in life, there will be donor hair available to be used in the future. It is also useful for patients with curly hair, patients with limited donor hair and patients with advanced hair loss. Also there is no shaving involved in the donor area, which means there is less down time and the area can be easily camouflaged while it is healing.
In addition to being a superior method for harvesting grafts, the hair restoration surgeon can use trichophytic closure to repair scars left by previous surgeries. Older closure methods can leave wider, uneven, visible scars that can often be repaired using this advanced technique. While most patients are candidates for this method of closure, the hair restoration surgeon should determine if this method best fits the needs and concerns of the client individually. Due to the evolution of FUT hair transplant harvesting technique and all the advantages of this method to FUE harvesting, in the recent ISHRS meeting this technique was chosen as the Gold standard for hair transplantation harvesting by many experienced hair transplant surgeons.
